What is the Family Care Team Resource Hub?

The Family Care Team Resource Hub was created from a partnership between Quality of Care NL, the Office of Professional & Educational Development in the Faculty of Medicine at Memorial University, and the Department of Health & Community Services with advisory input from Newfoundland and Labrador Health Services and other stakeholders.

If you’ve landed here, you’re working in, or are interested in working in, a Family Care Team in Newfoundland and Labrador. We’re happy you’re here! The Family Care Team Resource Hub has one main goal: to support Family Care Teams in inter-professional collaboration. As a health care provider, you may or may not have educational or career experience with team-based primary health care. As our province continues to move toward the delivery of primary health care in the Family Care Team model, we aim to provide tools, resources, and learning to support you working together.

Content for the Resource Hub evolved from a robust needs assessment carried out in the fall of 2023. We want you to know that we consider this site an ongoing needs assessment. You can contact us directly here. We want to hear your feedback on your learning needs, challenges, and successes so that we can continue to provide meaningful and timely tools and resources for you as part of a Family Care Team.

The materials and mechanisms available on the Resource Hub focus on inter-professional collaboration (IPC) and continuous quality improvement (CQI) and are designed for use by all health care providers working in Family Care Teams. Resource Hub content will evolve over time and you can expect to find learning and training opportunities to support your work in Family Care Teams.
