Bridging Health and Community - The Role of Family Care Teams in Addressing Social Determinants of Health

This 1-hour webinar will explore the social determinants of health (SDOH), emphasizing the critical role family care teams play in addressing these factors. Participants will learn about the impact of SDOH on health outcomes and discover how local social prescribing initiatives can bridge gaps in care by connecting patients with community resources.   3. Access and use additional resources from other external sources to support collaborative practice in Family Care Teams.

Family Care Teams: Effective Communication and Aligning Teams for Performance

In this webinar, you will explore some of the barriers to effective team performance. Case examples will include concrete tools and strategies to address conflict and improve communication to overcome these barriers.


This session will develop participants with skills to:

  • identify key leadership capabilities needed to align teams
  • create psychological safety for effective communication
  • apply new tools to improve communication
  • address conflict by understanding the interplay of power, trust and action

Quality of Care NL: Healthy Discussions - Family Care Teams

Family Care Teams are interdisciplinary teams that work collaboratively to best meet the needs of patients and families. This webinar features leaders from NL Health Services and the Department of Health and Community Services for a discussion on the implementation of Family Care Teams throughout the province and how it will improve the way we access primary health care in NL.
